The Fyra trains from AnsaldoBreda have never been inspected individually. Signs that went wrong things have been ignored in manufacturing. The inspections were conducted were mainly paperwork. That turned out today from the interrogation under oath for the Fyra committee of the House.
The commission today spoke with Krijn van Herwaarden, an Environmental Inspection Inspector and Transport (ILT), his boss, Inspector General Jenny Thunnissen and Jerry Borger, certification manager at Lloyd’s Register Netherlands. “The certificate may not be issued right and license nor” summed up committee Madeleine Toorenburg interrogations afterwards together.
Van Herwaarden told the commission that he had received signals that something was wrong with Fyra trains example of ProRail and his Belgian colleagues. However, he did not even bother to get in touch with his Belgian colleagues. His own colleagues warned him, after test drives in 2012. NRC-editor Jos Verlaan, who was present at the hearings today:
“They warned burned circuit boards. 45-volt lines that suddenly fell down. And doors that separate shots while the train was at full speed (250 kph). Whereby at least once narrowly avoided could be a technician from AnsaldoBreda was sucked out.
A colleague advised to cancel the order. But Van Herwaarden was adamant:
“I do not act on rumors. (..) I have not called those colleagues. I’m not train, I can not cancel material at all. “
These things belonged in a test phase, was his assessment additionally. Verlaan:
“It was typical of his role in this matter. The trains he had never physically examined. He did not need to. He relied on the audits and reports from Lloyd’s Register Netherlands, a certification agency that since 2004 has audited under contract to AnsaldoBreda or the manufacturer in the design and construction of the trains continued to laws and regulations. That homework he did not go again sometime. “
Van Herwaarden’s boss, Inspector General Thunnissen, gave him all the space, as shown by her interrogation. Because Thunnissen had Fyra project entirely up to him. Everything was “mandated by”. And as inspector general could they not know everything.
“I’m not an inspector, he’s the expert.”
Thunnissen and Van Herwaarden also had little visibility on how Lloyd’s checked the manufacturer. As Jerry gave Borger, certification manager at Lloyd’s, already understand today.
Borger came once a year since 2004 and never saw strange things in there. Verlaan:
“That was hardly his visits were announced in advance, including information about what he was going to check. And Ansaldo was his client, who also determined how heavy those controls might be. So given that Ansaldo that the trains are not physically one by one could be examined. Lloyd’s should examine in particular the administration and quality management. And who were all those years, according to Borger in order. “
Borger reported his findings to the ILT. They were then the foundation for the July-issued permits and clearances. Yet Van Herwaarden still felt he had not been “paper-stamper.
“I’m still behind my signature in July 2012.”
The V250 trainsets AnsaldoBreda finally delivered with much delay had to quickly be removed from the track because of serious flaws: the trains were destroyed in icy conditions. How has it come to one of the questions currently investigating the parliamentary commission of the Lower House. In total, up to 12 June heard forty people
